Syndemosis Injury Case 1

Pronation/External Rotation (Weber c) ankle fracture. Mortise view shown demonstrates medial clear space widening, tibiofibular clear space widening, loss of tibiofibular overlap: all indicating a Syndesmotic injury associated with the ankle fracture. Fibular shortening with lateral subluxation of the talus is also demonstrated.

Lateral view.

Patient underwent ORIF of the lateral malleolus. Post operative Mortise view demonstrates continued medial clear space widening, tibiofibular clear space widening, loss of tibiofibular overlap. The fibula remains shortened with lateral subluxation of the talus.
Fixation was revised, adding a syndesmotic screw. Post operative Mortise view demonstrates continued medial clear space widening, tibiofibular clear space widening, loss of tibiofibular overlap. The fibula remains shortened with lateral subluxation of the talus.
Patient was referred and eventually fixation was revised with restoration of fibular length and reduction of the syndesmosis. Mortise view shown.
