Case 2

30y/o male sustained a Mason Type III radial head fracture in a fall from high. Lateral injury view shown.

Oblique injury view.

Injury was approached via a Kocher lateral approach to the radial head.

Kocher Lateral Approach to Radial Head
  • Kocher lateral incision centered over Radial head.
  • Interval between the anconeus and extensor carpi ulnaris opened. (Consider opening the white fascial layer which defines the extensor origin down to bone, going through the extensor origin, annular liagment and LCL complex as one unit.)
  • ECU elevated exposing the lateral collateral ligament.
  • Incise capsule anterior to the LCL complex. Exposing the radial head.
  1. Skin incision: Lateral incision centered over radial head.
  2. Extensor carpi ulnaris
  3. Radial Head
  4. Anconeous
  5. Capitellum
