Crossover Toe Correction Pre-op Planning

  • Ensure appropriate fixation and instrumentation is available.  Consider Arthrex Complete Plantar Plate set.

Crossover Toe Correction Technique

  • Pre-op antibiotics
  • Sign operative site
  • All bony prominences well padded.
  • Anesthesia
  • Prep and drap
  • Dorsal incision centered over 2nd MTP joint.
  • Dissection done under 2.5x magnification.  Extensor tendon preserved and mobilized laterally. 
  • McGlamry Elevator utilized for mobilization.
  • Weil osteotomy performed, and capital fragment pushed back with the metatarsal pusher 8–10 mm and pinned with a 0.062 K-wire.
  • Second 0.062 K wire placed in proximal fragment. 
  • Small Joint Distractor placed and plantar plate exposed. 
  • Plantar plate released from the base of the proximal phalanx as close to the insertion of the proximal phalanx as possible.
  • Mini Scorpion used to place 2 sutures in the plantar plate in an inverted mattress configuration.
  • The distractor removed as well as the distal k-wire.
  • Two parallel holes placed with a K-wire from anterior to posterior in the base of the proximal phalanx with the K-Wire exiting just below the plantar cartilage.
  • The k-wire removed from the metatarsal and the capital fragment from the prior osteotomy positioned and secured with a QuickFix screw.
  • The toe placed in 10-15 degrees of plantar translation and the the planter plate sutures tied over the base of the proximal phalanx firmly restoring the plantar plate.
