    Focal Periphyseal Edema (FOPE) Etiology / Epidemiology / Natural History

    • Etiology is unknown.
    • Likely relates to the early stages of physiologic physeal closure with repetitive microtrauma.
    • Fope may be associated with pain particularly when no other MRI abnormalities are present.
    • Patients often, but not always involved in in athletic activities.

      Focal Periphyseal Edema (FOPE) Clinical Evaluation

      • Generally normal knee exam.  May have tenderness along the affected physis. 

      Focal Periphyseal Edema (FOPE) Xray / Diagnositc Tests

      • FOPE MRI findings: focal bone marrow edema pattern centered at the physis of the distal femur (most common), proximal tibia, or proximal fibula and extending into both the adjacent metaphysis and epiphysis.

      Focal Periphyseal Edema (FOPE) Classification / Treatment

      • Treatment: symptomatic, consider non weight-bearing for 4 weeks, bracing or physical therapy.
      • FOPE requires no invasive diagnostic procedure, and does not need imaging follow-up.
